Self ReferralTo refer yourself to Kedesh, all you have to do is phone our Admissions Centre. You will need to complete a Phone Assessment with us to apply: this usually takes about fifteen to twenty minutes to complete. After you have done the Phone Assessment, you will need to phone Kedesh the next day to find out the outcome of your assessment. Before you can be put onto the waiting list to enter the programme, we may need to get some additional information from you. We’ll tell you if we need any when you call back after your assessment. WorkersTo refer a client to Kedesh, you can phone our Admissions Centre with the client to complete a Phone Assessment, or the client can phone by themself to apply. To ensure continuity of care, please complete a release of information form with the client and fax it to our Admissions Centre. This will allow us to keep you informed of your client’s process both prior to, and after, admission to Kedesh. DoctorsIf you have a patient who has applied to Kedesh, we may need some additional information or assistance from you to facilitate their admission and ongoing treatment. If the patient is currently taking medication, or has a current psychiatric diagnosis, we will contact you for the relevant details. The commonly required forms are available below. Patients on medication:If your patient is currently taking any medication, we will require you to complete our medication chart, outlining all current medications, before the patient can be admitted. If this is required, we will send the medication chart to you, and request that you complete and fax it to our Admissions Centre. Patients with psychiatric diagnoses:If your patient has a co-occurring psychiatric condition, we will require some information from you about their condition and treatment history, before the patient can be admitted. We will send you a psychiatric form, and request that you complete and fax it to our Admissions Centre. Detox:Before your patient can be admitted to Kedesh, they will be required to be completely detoxified from all of the following:
If your patient wishes to do this under your supervision, we will require them to complete weekly urinalysis for drugs and alcohol, and for you to forward the results to our Admissions Centre via fax or mail. In addition, we will require you to provide written confirmation that they have been detoxified, in the form of a letter dated within the 48 hours before their admission to Kedesh.
